چکیده

This issue of Sleep Science contains two excellent review articles by Garcia-Rill and his colleagues examining the role of reticular activating system, especially the pedunculopontine (PPT) nucleus, in the pathophysiology of insomnia, schizophrenia, and other neuropsychiatric disorders [1,2]. The PPT nucleus is one of the major aggregations of cholinergic neurons in the mammalian brainstem capable of synthesizing acetylcholine, but also other neurotransmitters and peptides [3]. In addition, PPT cholinergic cells also synthesize a gaseous neuromodulator, nitric oxide. Interestingly, cells in the PPT are involved in regulation of both wakefulness and REM sleep [3,4]. About 10 years ago, the Garcia-Rill lab reported for the first time the presence of electrical coupling through gap junctions of some cells in each of three major elements of the arousal system, including the PPT [5]. This discovery helped explain how gap junction blockers like halothane and propofol can put us to sleep so rapidly. A year later, his lab resolved a 20-year mystery, the mechanism of action of the stimulant modafinil used in the treatment of narcolepsy. Modafinil was found to increase electrical coupling and coherence of cell activity in the same centers found to manifest electrical coupling, increasing arousal [6]. All of these breakthroughs

Pedunculopontine arousal system physiology – Deep brain stimulation (DBS)

This review describes the wake/sleep symptoms present in Parkinson׳s disease, and the role of the pedunculopontine nucleus in these symptoms. The physiology of PPN cells is important not only because it is a major element of the reticular activating system, but also because it is a novel target for deep brain stimulation in the treatment of gait and postural deficits in Parkinson׳s disease. A g...

Pedunculopontine arousal system physiology—Implications for schizophrenia

Schizophrenia is characterized by major sleep/wake disturbances including increased vigilance and arousal, decreased slow wave sleep, and increased REM sleep drive. Other arousal-related symptoms include sensory gating deficits as exemplified by decreased habituation of the blink reflex. There is also dysregulation of gamma band activity, suggestive of disturbances in a host of arousal-related ...

Evaluation of nicotinic receptor of pedunculopontine tegmental nucleus in central cardiovascular regulation in anesthetized rat

Objective(s): Cholinergic neurons are important neurons in the Pedunculopontine tegmental nucleus (PPT). In this study, nicotinic receptor of the PPT in central cardiovascular regulation in the anesthetized rat was evaluated. Materials and Methods: Saline, acetylcholine (Ach; doses: 90 and 150 nmol), hexamethonium (Hexa; doses: 100 and 300 nmol) and higher doses of Hexa (300 nmol) + Ach (150 nm...

Pedunculopontine arousal system physiology – Implications for insomnia

We consider insomnia a disorder of waking rather than a disorder of sleep. This review examines the role of the reticular activating system, especially the pedunculopontine nucleus, in the symptoms of insomnia, mainly representing an overactive waking drive. We determined that high frequency activity during waking and REM sleep is controlled by two different intracellular pathways and channel t...

Cardiovascular effects of nitrergic system of the pedunculopon-tine tegmental nucleus in anesthetized rats

Objective(s): Nitric oxide (NO) is an important neurotransmitter in central nervous system involved in central cardiovascular regulation. The presence of NO in the pedunculopontine tegmental (PPT) nucleus has been shown, but its cardiovascular effect has not been determined. In the present study, the cardiovascular effect of NO in the PPT nucleus was evaluated. Materials and Methods: After indu...
